| To register as a patient |
1. Please telephone our Registration Desk to see if we are taking any new patients.
2. Ensure your place of residence falls within our catchment area (see map below).
3. Download and complete both of the following forms. Form 1 Form 2
4. Please bring forms to the surgery to complete the registration application process together with the documents listed below
Please note that you will be required to produce documentation to support your application for registration:
1) Medical card - if you are unable to produce this, you will be asked to complete GMS1 form which must be completed with all past registration details. This enables your registration to be processed correctly
2) Proof of identification - for example, birth certificate
3) Utility bill to support your place of residence
4) Evidence to support your residence in this country, if necessary
As part of the registration procedure, new patients are asked to complete a medical questionnaire identifying a past medical and family history, details of medication and immunisations. Patients will also be asked to have a new patient medical check.
