  Medical Certificates
If you have been off work less than 7 days you do not need a sick note from a Doctor. You have to obtain a self certification note (SC2) from either your employer, the Post Office, Dept of Work and Pensions (formerly DSS office), or click here. If you are unemployed or self-employed please fill in a SC1 form.

If your employer will not accept the SC2 form, you can either:

- Tell your employer to phone us, so we can explain the legalities
- Print off this letter and give it to your employer (Download letter)
- Pay £10 for a private medical note (please enquire at the surgery).

If you have been off work for more than 7 days please complete the form located here and the Doctor will write a certificate. The Doctor will be checking your records for the appropriate medical details and will excercise their professional judgement if there are any issues. They will contact you if required.

Ongoing Certificates
Please complete the form located here if you are requesting an ongoing sick note (i.e. you have had continuous sick notes from us).

Other Useful notes
You do not neccessarily need to see the Doctor on a face to face basis to get a sick note. As long as the Doctor has enough evidence of the reason for sickness, the notes can be done (e.g. hospital letter, casualty attendance - but not the first week of illness).

Hospital Doctors are allowed to issue sick notes before you leave. Therefore please ask the duty staff to organise this.
